Failure Point 1: The wrong anesthesia. General anesthesia during BBL is the single largest driver of fatalities globally. When a patient is unconscious and a machine is breathing for them, the risk of fat embolism — fat entering the bloodstream and reaching the lungs or heart — increases dramatically. The standard of care has shifted globally toward awake anesthesia (tumescent local anesthesia) precisely because it removes this risk almost entirely. Yet many clinics in Lagos still use general anesthesia — because it is more convenient for the surgeon, not safer for the patient. If a surgeon cannot tell you clearly why they use awake anesthesia and demonstrate that it is their standard practice, you have your answer.

Failure Point 2: Incorrect injection depth and technique. Fat must be injected into the subcutaneous layer — beneath the skin, above the muscle. Injecting into or below the gluteal muscle puts the fat in proximity to large blood vessels. This is where fatal fat embolisms originate. This is a technique error, and it is not made by surgeons with genuine training and thousands of repetitions. It is made by people who learned the procedure watching a few videos or assisting someone else for a short period and then decided they were ready to operate independently.

Failure Point 3: Unqualified practitioners. This is the one that makes the industry genuinely dangerous in Nigeria right now. There are people performing BBL procedures in Lagos today who have no surgical training whatsoever. In some cases they are nurses. In some cases they are individuals who had the procedure done on themselves and concluded that watching it happen qualifies them to perform it. They operate in apartment clinics, hotel rooms, and backstreet facilities. They charge significantly less than proper surgeons. What Botched BBL Actually Looks Like

Not every bad outcome is a fatality. The majority of BBL complications are results — visible, lasting, costly to correct. Here is what they look like in practice:

• Asymmetry — one side noticeably larger, higher, or shaped differently from the other. Usually caused by inconsistent fat injection technique.
• Hard lumps under the skin — fat that has calcified or been injected unevenly, creating visible and palpable irregularities.
• Skin irregularities and dimpling — caused by aggressive or careless liposuction that removes fat too close to the skin surface.
• Results that disappear — fat transfer survival rates depend heavily on technique. Poor injection technique results in a high percentage of the transferred fat not surviving, leaving the patient with little to no visible result after months of recovery.
• Infection — from unsterile facilities or inadequate post-operative care.
• Severe scarring — from poorly placed incisions or inadequate aftercare.
